The first thing Dr. Collis will do is have a consultation with you about the health complaints about which you are primarily concerned. He will also want to discuss your family history, physical and work activities. The doctor will want to know what other care you may have had (chiropractic, physiotherapy, RMT, medical, etc.) and other questions designed to help determine the best treatment plan for you.We request that all new patients fill out our patient forms. The paperwork provides us with general information about you and will allow your consultation to be more thorough.
Please download and print the form found at the bottom of this page.
After discussing your concerns with Dr. Collis, he will perform a full chiropractic examination, checking standard neurological, orthopedic, postural, and physical tests.
During the exam, the doctor will explain his findings, and its significance to your case.
While we do not take x-rays in our office, Dr. Collis may sometimes recommend them. If they are recommended, we will send you to a nearby x-ray lab. If you have recent previous x-rays bring them in so that Dr. Collis may review them.
After your patient forms are completed, you will have a consultation with the doctor to discuss your condition, concerns, and treatment options. Your first chiropractic visit is designed for Dr. Collis to learn more about you, your overall health condition, and your expectations. A specific plan will be determined by Dr. Collis regarding your condition. Treatment can often be given on the first visit.
